When you arrive at our office, you will be greeted by one of our receptionists and asked to complete some paperwork.
You should arrive at the office approximately 10-15 minutes before your scheduled appointment time.
Personal and medical history: Please bring with you the name and telephone number of your medical doctor or clinic.
If you have had recent surgery, we will also need to know the name and telephone number of your surgeon.
Also, bring a list of any prescription or over the counter medication that you may be taken.
